At Legacy of Care Health Center, our mission is to provide free medical care and health promotion services to all uninsured and underinsured adults through disease management, preventative education and health screening by volunteer professionals for the purpose of building a healthy community. That includes our vision of increasing access to a continuum of quality health care services.



Health Care



Legacy of Care Health Center provides comprehensive healthcare services to the uninsured and underinsured. Our health care providers demonstrate high levels of responsibility and accountability. Although our services are available to the general public by appointment, patients must provide documented proof they need our services.



Primary Care



Services provided at the clinic and in the community include: Basic Primary Care, Physical Examination, Prescription Medications, Laboratory Testing, Diagnostic Testing, Specialty Referrals, Cancer Screening ( Cervical, Breast, Colorectal, Prostate) Patient Education
